Job Description
In
collaborative practice with the UCLA Oncology care team, including attending
physicians, fellows, treatment team nurse practitioners, nurses, and Ambulatory
Operations staff, the Oncology Medical Home (OMH) Nurse Practitioner will be
responsible for providing care management services for oncology patients to
facilitate an oncology medical home. The OMH Nurse Practitioner is a Master's
prepared oncology advanced practice nurse practitioner who will assist in
direct patient care coordination, care management, care navigation, and patient
care. The OMH Nurse Practitioner will be responsible for communication,
coordination, collaboration, and feedback with treating physicians, nurse
practitioners, patients, and families/caregivers as a care extender of
attending Oncologists. The OMH Nurse Practitioner will receive and place calls/virtual
visits with high risk oncology patients
that will involve assessment of patient needs and provision of appropriate care
and care coordination to ensure positive
patient outcomes. The OMH Nurse Practitioner will review internal and health
plan-reported performance metrics and patient detail data and conduct chart
reviews to understand high risk drivers, to understand patient care
trajectories, and identify high risk patients proactively. The OMH NP will provide
and facilitate care management activities to enhance the quality and
coordination of care. Goals will include prevention of avoidable
hospitalizations and emergency department visits, symptom management,
facilitating end of life planning and care. The OMH NP will participate in care
design, quality improvement, and process improvement activities. The OMH NP
will coordinate care with the broader medical neighborhood including primary
care, other specialty care, and support services.
The Nurse Practitioner functions in an interdependent
role by implementing the appropriate 'Standardized Procedures' (SPs). By using
the SPs, the NP is able to change treatment regimen after observing signs and
symptoms of illness, reactions to treatment, general behavior and physical
condition, and determines if they are abnormal findings (CA Nursing Practice
Act, subsection(b) of Section 2725).
- Current California Licensure as RN and Nurse Practitioner - required
- Graduated with an MSN/DNP Specialty in NP Specialty: AG-ACNP, AG-PCNP, FNP or Adult NP with oncology experience - required.
- Advanced Oncology Certified Nurse Practitioner (AOCNP) -preferred
- National NP certification in Specialty- required
- Previous NP experience- required
- Oncology experience as an RN or NP - required
- Current BLS from ARC or AHA - required
- Skills and abilities to perform accurate assessment and evaluation oncology patients, including symptom management - required
- working knowledge and understanding of various insurance plans- preferred
- NPI less than 6 months experience
