Requisition #10543 Assistant Director of Care Coordination (Health Care Integration Centralized Costs, Jamaica Plain) Schedule: Monday - Friday, No weekends, No On-call Salary: $80,000-$85,000

    The Assistant Director partners with the Director to provide oversight of Behavioral Health Care Coordination as implemented through CP Care Teams in the Greater Boston, Southern and Northern Regions. This role directly supervises Team Leaders of a cluster of Care Teams in the provision of intensive care coordination and clinical care management for Mass Health Members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. Therein, the Assistant Director is responsible for hiring, training, supervising and evaluating Team Leaders. The role is responsible for the development and implementation of care team policies, procedures and work flows so as to ensure that each Care Team is meeting quality performance metrics and Enrollee satisfaction. In partnership with the Program Director, the Assistant Director collaborates with the Executive Director of CCP in developing and managing ongoing relationships with Enrollee’s ACO/MCO plans to minimize duplicative efforts, promote integrated care, ensure quality and continuity of care, and support the values of person centered planning, Community First and SAMHSA Recovery Principles.
    Job Duties and Responsibilities: The essential job duties/responsibilities of the position include but are not limited to the information listed below:
  • Oversee Behavioral Health Care Coordination as implemented through a cluster of CP Care Teams.
  • Supervise CP Team Leaders in the provision of care coordination / care management.
  • Develop and implement care team policies, procedures and work flows to ensure all Community Program functions are effectively met.
  • Hire, train, supervise and evaluate Team Leaders.
  • Liaise with Team Leaders in developing and maintaining ongoing working relationships with Enrollee’s ACO/MCO plans.
  • Consult with agency Medical Director with regard to complex clinical cases.
  • Collaborate with CCP team to analyze program data and implement quality improvements as indicated.
  • Serve as liaison to any or all of the following: the Department of Mental Health (DMH) Site/Center and Area staff; Behavioral Health Care Providers; local Emergency Services Team; Inpatient and Partial hospital staff; Guardian or family members, and other involved providers, as necessary or requested.
  • Promote the utilization of evidence-based and promising practices that optimize care integration and effective communication across systems of care, and that align with rehabilitation, recovery and wellness principles.
  • Ensure all Enrollees are consistently provided with trauma informed and cultural responsive services.
  • Ensure that the SDH needs of each Enrollee are being met.
  • Ensure for timely information exchange, coordination, and integration of care.
  • Typical Requirements: Minimum of 5 years supervisory experience required.
  • Clinical and case management experienced required.
  • Effective skills in managing, teaching, and negotiating, and in collaborating with multidisciplinary teams and client/family focus.
  • Effective relationship, management and team building skills.
  • Experience working with people living with SMI and/or SUD.
  • Preference given to bi-lingual/bi-cultural applicants and those with lived experience of BH conditions.
  • Education and Required Credentials/Licenses: Master’s Degree required; License (LPHA) to practice as "Licensed Practitioner of the Healing Arts” in MA; MA license to practice independently (LICSW, LMHC).
